The role:

We are looking for a Support Coordinator to join our supported living service for adults with learning disabilities, autism, and complex needs. The supported living service operates in both Andover and Alton and you will be responsible for the day to day coordination of the support and the development staff.

Your focus will be ensuring that the outstanding level of care set by the team is maintained and you will be responsible for the assessment and formulation of care plans, risk assessments and care files.

You will supervise and manage established teams of staff providing person centred support to enable service users to maintain their independence, experience improved wellbeing and enjoy opportunities for personal development.

We are looking for a role model for our care staff that can provide hands on support and will focus on addressing barriers and reducing complexity in the support environment.

As our Support Coordinator, you will:

  • Ensuring that all service users’ visits are supported with the relevant support workers appropriate for their needs, using appropriate systems and processes to manage all bookings.
  • Lead the day to day routines of the service, including being responsible for the rostering, leading, and organising the duties of staff.
  • Assist individuals to plan their life around their individual needs and goals ensuring these are assessed in relation to their lifestyle and taking account of risk management approaches.
  • Ensure that person centred plans are monitored and reviewed, capturing progress, and recording outcomes
  • Promote skills development including supporting people to maintain and develop links with family, friends, and other professionals across a wide variety of locations.

What we are looking for from you:

  • Previous experience in a similar role within a health or social care setting.
  • Experience working in a fast paced and constantly evolving environment.
  • Strong people management skills and able to remain clam under pressure.
  • NVQ Level 3 in Care
