The Role

Enham Trust are looking for a Service Manager to lead the Transition Service in exceeding the support standards determined both internally and by the local authority. Your role will be to develop, implement and lead a service that supports individuals with a disability to transition between home and independent living.

We are looking for someone with a passion for care to had minimal gaps and that all service users have up to date support plans, appropriately funded support packages and have set outcomes documented.

As our Service Manager, you will be responsible for:

  • Ensure that high standards of support are delivered to meet the needs of individual residents through the delivery of person-centred support services that promote independence, choice, and dignity to enable people to live, work and enjoy their life.
  • Take a multi stakeholder approach to service development by working in partnership with all internal and external stakeholders.
  • Ensure that all new referrals are responded to in a timely manner in line with contractual and organisational timescales. Negotiating support packages in line with organisational expectations.
  • In partnership with the Director of Care, develop, implement and manage an outcomes focused toolkit to track service user growth within the service.
  • Ensure medicine receipt, disposal and administration is handled and documented in line with policy and NICE guidelines and regulations.


What we are looking for from you!

  • Fully adept at being able to develop, review and maintain person centred plans.
  • Strong knowledge of safeguarding vulnerable adults and children.
  • Proven experience working within care / support setting or similar
  • Experienced in developing budgets, managing departments to these and reporting against budget
  • Managing difficult conversations and situations
