Hourly Rate: £12.46 Self Employed. £10.20 Employed Job Overview: To provide support and develop further independence with home, health and social care needs that will vary from week to week. Activities may include the following: • Checking client has opened post as provide support for any follow up required • Monitoring and assisting with any medication required• Providing transport and assistance with any medical or hospital appointments• Helping client to develop skills and confidence in managing household chores, cooking, laundry, and general organisation, and prompting for good personal hygiene• Supporting any social or sporting activities• Facilitating travel in the UK and possibly/occasionally abroad Client Biography: Young male adult in his mid twenties born very prematurely and recently diagnosed with Velo Cardio Facial Syndrome. The client has recently experienced very poor mental health and is rebuilding his life to manage this condition with support from the local EIP and adult social care.He has a desire for complete independence and enjoys, travel, writing, cooking, current affairs, walking and going to the gym. Whilst he is part of a large family and enjoys their company, he would like to develop his own friendship group, but finds this challenging.He is easy going and eager to please, but is quite determined and strong minded when something is important to him. Essential Criteria: • Similar age band or someone who the client can relate to• Ability to communicate, clearly, precisely, and repetitively on occasion to ensure understanding.• Experience of supporting a person with managed acute mental health together with elements of learning difficulty• The ability to see beyond the surface and patiently nurture the building of confidence and capability required for independent living. Type of experience required: An Enhanced Disclosure and Barring service (DBS) will be required the cost will be met by the employerLength of experience required: Minimum of 2 years working with a similar clientFull Driving Licence: Essential.
